is the
Theory of Change?
The goal of the CO-CREATE project is to eliminate the inequalities in testing, access to care, morbidity, and mortality from COVID-19, in the San Ysidro community. The Theory of Change process was created by our Community and Scientific Advisory Board using the knowledge and wisdom of community members. Their insights helped us more fully understand the factors contributing to these disparities and what types of action could be used to address them.
